Rules and procedure for providing charitable assistance and organization of public fund collection.

1. GENERAL PROVISIONS

1.1 In these rules, the following terms are used:

  • 1) The Found is a non-profit Charity Organization "International Charity Fund "Love to Ukraine".
  • 2) Charity recipient - receiver of charitable assistance (private entity, non-profit organization or territorial community) who gets assistance from one or more charity providers to achieve the goals defined by the Law of Ukraine "About Charitable Activities and Charitable Organizations".
  • 3) Charity recipients can be also any legal entities that receive assistance to achieve the goals defined by the Law of Ukraine "About Charitable Activities and Charitable Organizations".

1.2 The Fund performs its activities in accordance with the statute.

2. PROVIDING THE DOCUMENTS TO THE FUND AND PROCEDURE OF THEIR REVIEW:

2.1 The following documents are sent to the fund for consideration in accordance with the program:

  • humanitarian assistance
  • Copy the letter of inquiry (file);
  • Copy of passport (all pages);
  • Copy of INN;
  • Copy of the Internally Displaced Person (IDP), if available.
  • Permission for publication of the material;
  • Child's birth certificate, serial number
  • If you are a single mother and a single father, please provide a copy of the relevant documents.
  • medical assistance of militaries
  • Copy the letter of inquiry (file);
  • Copy of passport (all pages);
  • Copy of INN;
  • Copy of a military ticket;
  • Copy of the letter describing the history of the disease;
  • Сopy of the doctor's conclusion, X-Ray images, extracts;
  • Requisites of the banking account opened for the CHARITY RECIPIENT or the name of the medical institution;
  • Permission for publication of the material;
  • Amateur photos of the CHARITY RECIPIENT (Military man) (3-4 pcs.);
  • Copy of the disability certificate (if available, xerox copy).
  • medical assistance for children
  • Application from the father/mother (two applications from both parents are provided) (file);
  • Copy of passport (all pages);
  • Copy of INN;
  • Copy of the letter describing the history of the disease;
  • Сopy of the doctor's conclusion, X-Ray images, extracts;
  • Child's birth certificate;
  • Permission for publication of the material;
  • Application for funds collection;
  • Requisites of the banking account opened for the CHARITY RECIPIENT or the name of the medical institution;
  • Photo of child (3-4 pcs.);
  • If you are a single mother and a single father, provide a copy of the relevant documents;
  • Certificate about family membership (who is registered with the CHARITY RECIPIENT, xerox copy);
  • Xerox copies of certificates about incomes of all family members.

2.2 The CHARITY RECIPIENT is responsible for the reliability of all information provided to the FUND.

2.3 After approval of the documents mentioned above and verifying the information, the FUND makes a decision about the possibility of providing charitable assistance to the CHARITY RECIPIENT.

3. RIGHTS AND OBLIGATIONS OF THE PARTIES:

3.1 The FUND performs:

  • 1) Opening of the CHARITY RECIPIENT page in the FUND's official website.
  • 2) Translation of medical documentation into English language with the help of the FUND's partners (if the CHARITY RECIPIENTS have no possibility to translate).
  • 3) Performing of campaigns and promotions for the purpose of collecting funds in order to pay the CHARITY RECIPIENTS' accounts.
  • 4) Collection of expenses for payment of official invoices from clinics for operations, examinations, treatment, purchase of medicines or other items of a medical purpose.
  • 5) Collection of expenses for payment of the Charitable Organization for the CHARITY RECIPIENT through targeted advertising on behalf of the CHARITY RECIPIENT and using another methods.
  • 6) If necessary, the fund organizes photo and video session for further promotion of the collection for the CHARITY RECIPIENT.

3.2 The CHARITY RECIPIENT gives the right to the FUND to apply on his behalf to other funds and organizations with a request for assistance.

3.3 The initially discussed amount of the general collection is supported by the necessary documents from medical institutions. If unforeseen circumstances arise in the process of treatment, the amount of the total collection increases and is confirmed by the documents of the relevant medical institution.

3.4 The CHARITY RECIPIENT gives permission to place the photos, audio and video products in all means necessary for collection, particulary,  in the Internet.

3.5 The Fund has the right to pay for transportation and travel expenses related to the treatment of the CHARITY RECIPIENT and expenses for renting housing in a foreign country.

3.6 CHARITY RECIPIENT is obliged:

  • 1) to provide information about the health regularly, at least once every 3 days.
  • 2) to provide information based on the results of conversations with doctors, representatives of medical institutions.
  • 3) to inform the FUND about the changes in the choice of medical institutions, as well as in the tactics of fundraising.
  • 4) to inform the FUND about appeals to ministries and health care committees.
  • 5) At least once every 7 days, to provide information about the receipt of donations to the personal accounts of B CHARITY RECIPIENTS (including the extracts from the bank statements), receipt of cash from donators, postal transfers.
  • 6) With each spending of funds received for the purchase of medicines, payment for examinations, tests, operations, air tickets, supporting documents (commercial and cashier's receipts, copies of documents, transport tickets, etc.) must be provided to the FUND.
  • 7) to answer on calls and letters received from employees and volunteers of the FUND.
  • 8) to inform the FUND in written form if any of intermediary is involved in the treatment process:

4. SPECIAL CONDITIONS:

4.1 All finances that are collected by the FUND have to be paid on a special account provided by the CHARITY RECIPIENTS.

4.2 The FUND is not responsible for the independent collection of finances by the relatives of the CHARITY RECIPIENTS.

4.3 The FUND has the right on a unilateral basis to cancel the providing of charitable assistance in the following cases:

  • 1) When CHARITY RECIPIENTS and/or their relatives do not respond on letters and calls from employees and volunteers of the FUND.
  • 2) When CHARITY RECIPIENTS and/or their relatives perform independent fund-raising with declarations of needs and amounts that do not correspond to reality.
  • 3) When the CHARITY RECIPIENT does not provide information about the health conditions, the receipt of charitable funds, negotiations with doctors, changes and additions to treatment tactics, reports about the finances spent, in accordance with paragraph 3.6. In these cases, the Director of the Fund can on a unilateral basis refuse to provide charitable assistance and the same time close the collection or raise the issue at the meeting of the board of founders of the fund with the further official answer that will be sent to the CHARITY RECIPIENT by post or to e-mail address.

4.4 If the CHARITY RECIPIENTS refuse charitable assistance, treatment, consultation, medicines, surgery, etc., the FUND closes the collection of finances and makes an official statement on the FUND`s website about the closure of the collection. The finances from such collection will be transferred to other charity recipients who applied for help to the FUND and/or to the statutory activities of the Charity Organization "International Charity Fund "Love to Ukraine".

4.5 In the case if the collected finances are not spent for the certain purpose (for example, in case of death of the patient; difficulties in treatment), all collected finances are transferred to the account of the FUND, for the benefit of other charity recipients  who applied for help to the FUND and/or for the statutory activities of the Charity Organization "International Charity Fund "Love to Ukraine", which is certified by a corresponding letter addressed to the director of the Fund.

4.6 In the case if the amount of collected finances exceeds the amount of the organized collection, the FUND will transfer the extra finances to the benefit of other charity recipients who applied for help to the FUND and/or to the statutory activities of the Charity Organization "International Charity Fund "Love to Ukraine".

4.7 In case of non-targeted spending of finances by the charity recipients, the FUND has the right to submit information to the tax authorities.

4.8 The FUND provides assistance free of charge.

4.9 The total amount of collected finances is used in accordance with the statute, these rules and procedures, not taking into consideration the commission of payment systems.
